The Human Resource Management Office of DLSU-D is staging a second round of its Vacc to La Salle program to provide COVID-19 booster shots for employees of the University on January 27, 2022. The first event happened last January 13.
The vaccination drive will take place at the Animo Center, DLSMHSI to accommodate registered DLSU-D faculty and staff members, as well as a limited number of dependents.
Please be guided by the following steps to register for your slot:
Step 1: Pre-registration:
- Team HRMO Vax shall screen the eligibility of vaccinees.
- Qualified vacinees will receive the SMS and e-mail confirmation and the DLSUMC Vax Registry Link.
Step 2: DLSUMC Vax Registration:
- Vacinees must accomplish the DLSUMC Vacicnation Registry on or beore January 26 only.
Step 3: Booster Shot Vaccination:
- Proceed to the vaccination site on your assigned schedule.
The event is a partnership with the City Government of Dasmarinas, DLSMHSI, De La Salle University Medical Center, and De La Salle Philippines.
Reminders:
- No walk-ins
- Only employees who are pre-registered with confirmed schedule received through email and text will be accommodated on January 27.
- Due to the limited number of slots, only one (1) dependent per employee shall be accommodated on a first register first serve basis.
- DLSMHSI/DLSUMC, our Health Service Provider, shall charge a service fee for the logistics (i.e. vaccination materials, manpower, and other operations cost) of Php 350 per jab/inoculation and not as payment for the vaccine. This shall apply for dependents only and payment shall be through Salary Deduction.
